چکیده

Background Current heart failure guidelines recommend insertion of implantable cardiac defibrillators (ICD) for primary prevention of sudden cardiac death (SCD) in patients whose left ventricular ejection fraction is (LVEF) ≤ 35% on maximally tolerated medical therapy. However, the majority of sudden cardiac death (SCD) in patients with heart failure occurs in those with mild-moderate systolic heart failure (LVEF 36%-50%) who currently do not qualify for an ICD. At present, our tools to reliably risk stratify patients with mild-moderate heart failure to predict the likelihood of SCD are limited. Recent data however suggest that ventricular scar and/or replacement fibrosis, reliably identified on Cardiac Magnetic Resonance Imaging (CMR), forms a substrate for malignant arrhythmia, thus potentially identifying a group at increased risk of SCD. The primary aim of the Cardiovascular Magnetic Resonance GUIDEd management of mild-moderate left ventricular systolic Heart Failure (CMR GUIDE HF) trial is to test the hypothesis that among patients with mild-moderate heart failure, a routine CMR guided management strategy of implantable defibrillator (ICD) insertion is superior to a conservative strategy of standard care.

The Correlation between Left and Right Ventricular Ejection Fractions in Patients with Ischemic Heart Disease, Documented by Cardiac Magnetic Resonance Imaging

Introduction: The correlation between right and left ventricular ejection fractions (RVEF and LVEF, respectively) has been studied in only a small number of patients with a marked decrease in RVEF and LVEF. The aim of the present study was to compare LVEF and RVEF in patients with ischemic heart disease. RVEF and LVEF were measured by Cardiovascular Magnetic Resonance (CMR) imaging. Materials a...

CARDIOVASCULAR MEDICINE Effects of carvedilol on left ventricular remodelling in chronic stable heart failure: a cardiovascular magnetic resonance study

Background: The ability of b blockers to improve left ventricular function has been demonstrated, but data on the effects on cardiac remodelling are limited. Objective: To investigate, using cardiovascular magnetic resonance (CMR), the effects of carvedilol on left ventricular remodelling in patients with chronic stable heart failure and left ventricular systolic dysfunction caused by coronary ...

Left ventricular systolic function assessment in patients with dilated heart failure using cardiovascular magnetic resonance

Cardiovascular magnetic resonance (CMR) has become a reference standard for the measurement of cardiac volumes, function, and mass. This study aims to reconstruct three dimensional modeling of the left ventricle (LV) in patients with heart failure (HF) using CMRtools and thereby derive the LV functional indices. CMR images were acquired in 41 subjects (6 females) with heart failure (HF) and 12 ...

The Carvedilol Prospective Randomized Cumulative Survival (COPERNICUS) trial

Previous trials (Metoprolol CR/XL Randomised Intervention Trial in Congestive Heart Failure [MERIT-HF], Cardiac Insufficiency Bisoprolol Study [CIBIS] II) have demonstrated a mortality benefit of beta-adrenergic blockade in patients with mild to moderate heart failure. The recent Carvedilol Prospective Randomized Cumulative Survival (COPERNICUS) trial has extended these results to a more advanc...
